The 26-0083 Form Explained

FLIGHT CREW PERMIT / LICENCE
APPLICATION FOR ENDORSEMENT OF A RATING

 

Part A is only signed when all issuing requirements are met. It means the Applicant has flown the required hours and
has completed successfully the required flight test or PPC before he/she signs Part A. The best practice is to sign
Part A at the time the Authorized Agent or Inspector signs/endorses the Applicant's license.

You can notify Transport Canada of the applicant's change of adresse by writing here the new address along with a
visible note "New Address" on the first line near the surname. TC personnel will then issue and mail the new licence at
the new address. For better follow up purposes, some organizations prefer all licences be mailed to their corporate address
instead of the individual pilot home address. In this case write "c/o (compagny) Flight Operations along with the mailing address.
